We are currently accepting referrals for individuals seeking personal care services in Vance County and the surrounding areas. CareAssist is committed to providing dependable, compassionate support that families can feel confident about.
Coverage & Payment Options.
✓ We accept North Carolina Medicaid and Medicaid-managed plans (including Medicaid through major insurance providers).
✓ For individuals without Medicaid coverage, flexible private pay options are available.

How to Get Started:
Getting started with personal care services is simple. Our team is here to support you every step of the way.
Download the Personal Care Services (PCS) Referral Form (click here).
Your physician completes and submits the referral documentation to Acentra Health.
Acentra Health reviews your information and determines eligibility for Personal Care Services.
Once approved, select CareAssist, LLC as your service provider.
Our team coordinates next steps and begins your personalized care plan.
